What is eForms Platform?

eForms solution is a digital forms repository, tightly integrated into EMRs, capable of pre-populating patient/provider information into digital forms.​

What is a Limited Production Release?

Ontario Health is piloting the eForms platform to assess clinician user experience with the digital Health Assessment Form (HAF). The goal is to gather feedback from primary care physicians and clinicians to help refine and optimize digital workflows ahead of a province-wide rollout. Participation in the Limited Production Release (LPR) involves a total time commitment of approximately 3–5 hours, including onboarding, form completion, and feedback sessions.

What is in it for me?

  • Participating physicians and clinicians will be compensated for their time.
  • Help shape and improve digital workflows for physicians and clinicians across Ontario.
  • Provide valuable insights, especially if you work with Indigenous patients or communities.

Who can participate?

  • Primary care physicians and clinicians who serve Indigenous patients and submit the Ministry of Long-Term Care (MLTC) Health Assessment Form (HAF) for long-term care admissions in the Toronto Central or Central East regions (the two participating regions in the LPR).
